Concierge Care Management

Providing four hours of care management a month, the goal is to address an immediate crisis and to address the short term goal. Should the client choose to continue this option for more than three months, the care plan will be updated.

Services Offered

Consultation with the family members to assess health and safety of the client.
Support during times of crisis.
Guidance and assistance with transition during moves. Referrals to trusted community professionals.
Support with implementation of the care plan; fostering family engagement,
providing confidence and peace of mind.
